Holothrix filicornis bears two heart-shaped to rounded leaves positioned flat on the ground, one overlapping the other at the base. The leaves are succulent, about 3,5 cm in diameter. Curving pale veins are visible on the faintly glossy green blades in picture.
There are no stem leaves on the scape, although tiny bracts are present among the flowers. The scape appears to be hairless (Williamson, 2010).
